If you performed a factory-reset on your device, reinitialized it, and were already using it after that, chances are very low. Having too much money (or the stuff being really worth it), forensics might have a chance – but apart from that, nothing an average mortal could do. OK, that's definitely the drawer. Please try the following: Go to your Android settings, chose Apps › All, scroll to "Photos". Is the entry duplicate here as well? If so, try opening one, and see whether you can disable it. Does the second one stay, or gets disabled along? Also worth a try: Check whether the app drawer offers to sort its entries – e.g. sort it by install-time, and then back to alphabetically. If it's really a dupe entry, that might remove it.

@Andruseto nonsense. Name a single virus for Android (hint: there is none). Things like the OP described might happen when the device runs out of power during an OS update.

Not really answering your question, but closely related: Can Google's Voice Actions be started by voice? The only thing I know in this context is an app called AutoVoice, which offers a "continuous mode" to be "permanently listening".

Unknown sources are always dangerous if you don't know whether you can trust them. And not everybody has a second device available. But of course, if you have that option, that might work. One site with links to .apk files (hopefully trustworthy) is mine ;)

Copying the .apk to your SD card means you've got to get hold of it in the first place – which is not always possible, as Playstore doesn't feature a "direct download" ;) BTW: I've added the remaining facts to your answer, hope that's OK with you. Full ack to the SD card class recommendation, of course – which is the same I usually give :)

That's for native App2SD. Starting with Android 4.0, many manufacturer dropped support for this from their devices – so it might be there, or not. If it's supported, that's of course the way to do it :)
