One more note: If my guess was correct, and your devices run 4.4.2, you might have hit a Kitkat bug (Google Translate version). The work-around described there is pretty much the same you're already using.

If you can live with "making your tab virgin again", I recommend checking our factory-reset tag-wiki for some first aid. Sounds really strange: simply plugging in a USB cable shouldn't do that (never heard such).

You are probably right about the app not triggering the mesia scanner. However, a reboot should trigger it. Let me make a guess: all affected devices already run Kitkat? Because starting with that, media scanner does no longer do a full scan AFAIR (at least the apps triggering it no longer can do so in the "old way"). If I remember correctly, media scanner must now be pointed to the changed files directly.

I don't remember ever having used fastboot, but I have CWM on some of my devices. So while some certainly require flashing it via fastboot, there must be other means; but I'm not aware of any such method not involving root :) +1 from me :)

No, I agree. I don't think any of the two services keeps a (publically available) database of "historical locations" (though Google might have such a "secret database", who knows ;)

I partly disagree with dotVezz here. It might very well depend on your web server handling the PDF-sending. Make sure you send the correct header info; if the server e.g. uses "octet-stream" instead of "application/pdf", it might go straight to downloads (unless the browser determines the correct mime type by the file's extension). But that's rather a question for Server Fault I guess :)

@dotVezz That's not a comment, but an answer, go ahead posting it as such :) Maybe a hint to custom recovery wouldn't hurt either, as that might require root permissions to be installed (and on most devices is the pre-condition to flashing custom ROMs).

Why it doesn't turn up immediately is easy: that requires the media scanner to add its name/location to its database. But a reboot should have triggered that definitly, so something seems to be wrong there indeed. How did you add the file? Have you tried doing so via MTP/UMS (while the phone is connected to your computer)?

According to XDA: Press down the keyboard "S" key (yeah S for Safe mode) at HTC logo during boot. As I have no Desire-Z, I cannot test myself; so please report back if that works, so I can update the tag-wiki accordingly :)

"Wipe Dalvik" usually is only available with custom recovery (ClockworkMod, TWRP, etc.). Have you tried the safe-mode tip? Just removing the updated app then might already solve the issue.
