Yes, fastboot -w erases both user data and cache, which is the same as the wipe/factory reset. That covers all of your data except for the SD card, which you'll have to place into another device or computer if you want to erase it.

@MatthewRead When the EUI-64 address is used, the IPv6 address becomes (in theory) unique to the device. It identifies the device and possibly the manufacturer (of the NIC, at least). Though, so does the User-Agent string, and nobody cares much about those because they don't necessarily uniquely identify the device.

Sorry, I know it's bad news, but even so, the computer might be able to read it. Make sure the card reader you pick up can handle SDHC cards.

Right. First specify exactly which phone you have. The one you specified in your tags doesn't have an SD card slot, and you couldn't possibly have removed an SD card from it.
