The app you are looking for is cLock. Its a widget and its still present in CyanogenMod.

Unfortunatelly, from Lollipop onwards, you can now get this onlycan't put any widgets on your homelock screen, becauseand by that you are unable to use the described widget as of CM12/Lollipopyou used it before.

Hovever, you can't add custom widgets tocan still place it on your lockhome screen, the default clock & date are shown.
